everyday LIFE Services

The everyday LIFE Day Health Center offers one convenient location where the member can go to receive preventive and restorative health care, meals, recreation and monitoring. The everyday LIFE Team works cooperatively with the member and his or her caregivers to formulate the optimal plan for maintaining the elder in the community for as long as possible.

The everyday LIFE Team assesses the needs of the member and delivers all services through our own staff or through contracted providers.

The everyday LIFE Team is comprised of:

• Primary Care Physician
• RN
• Physical Therapist
• Driver or Representative
• Personal Care Attendants
• Home Care Coordinator
• Medical Director
• Dietician
• Occupational Therapist
• Social Worker
• Recreational Therapist
• everyday LIFE Center Manager

The needs of an enrolled member will be assessed and a plan of care will be written by the             everyday LIFE Team and approved by you and your caregiver.

• Durable Medical Equipment

Eligibility Criteria:

• A resident of the everyday LIFE service area (Northampton and Lehigh counties).
• At least 55 years of age.
• Meet the medical eligibility for the nursing home level of care.
• Be able to safely live in the community with everyday LIFE services.
